• 04/23/1999 -- Approved/LSN (Approve the Special File Surcharge Supplement per LSN authorization on April 24, 1999 and to expire May 31, 1999. The temporary surcharge will be adjusted monthly to become effective the first day of each month thereafter until fuel prices have moderated. This will require the company to file monthly reports and adjusted surcharge supplements 30 days prior to the adjusted surcharge effective date (i.e. a one month lag in recovery) These reports will show the average fuel price for the current month and the resulting new surcharge amounts rounded to the nearest quarter for door to door stops and scheduled passengers for the following month, current revenue generated under the surcharge (including the number of door to door stops and scheduled passengers carried), and other information that the staff may direct. Require the company to comply with customer notification rules for subsequent updates by posting a notice on all company operated vehicles concurrent with the filing made with the Commission.
