Summary: Change affecting submission of annual performance assurance plan (PAP) payment cap calculation under Qwest's PAP, Section 12.1. As the result of an order granted by the FCC, the accounting data needed to make the annual return calculation for Section 12.1 will no longer be available. Qwest payments currently equal .074 percent of the calculated amount. Accordingly, the annual return calculation will be frozen at its current level but will be recalculated if future payments approach the cap amount. The PAP was originally approved in Docket UT-003040.
